Key Responsibilities- Assess the physical, psychological, social, and spiritual needs of patients and families within 24 hours of admission.
- Develop and implement individualized care plans to meet the unique needs of each patient.
- Collaborate with the interdisciplinary team to ensure seamless care coordination and communication.
- Provide education and support to patients, families, and caregivers on hospice care and end-of-life options.
- Administer medications and treatments as prescribed by the attending physician.
- Participate in quality assurance and performance improvement initiatives to enhance patient care and outcomes.
- Maintain accurate and timely documentation of patient care and services provided.
- Communicate effectively with patients, families, and healthcare team members to ensure a positive and supportive care environment.
- Associate of Science in Nursing (ASN) or Bachelor of Science in Nursing (BSN) degree.
- Active Registered Nurse license to practice in the state of Florida.
- 1-2 years of nursing experience, preferably in hospice care.
- Certification in Hospice & Palliative Medicine (CHPN) preferred.
- Strong communication and interpersonal skills.
- Ability to work independently and as part of a team.
