We are seeking a highly skilled and compassionate Senior Patient Care Manager to join our hospice team at SouthernCare. As a key member of our leadership team, you will be responsible for the day-to-day operations of our hospice site, ensuring the provision of high-quality care to our patients and their families.
Key Responsibilities- Administer the clinical aspects of the hospice program, ensuring compliance with regulatory guidelines and managed care principles.
- Manage and oversee the services and performance of assigned team members, including supervising, managing performance, and directing the work of specified team members.
- Participate in on-call nursing schedule and provide direct skilled bedside nursing care in patients' homes.
- Respond to phone referrals and inquiries in a timely, professional, and compassionate manner.
- Registered Nurse with ASN or BSN from an accredited school of nursing, one-year experience as an RN (Licensed in the state of requested employment).
- Three years' experience in direct patient care.
- One year management experience. Two or more years' experience in hospice or home care required.
- Must be able to effectively operate nursing assessment tools and office equipment to include, computer and or tablet.
- Sensitivity to feelings of the terminally ill and their families before and after a patient's death.
