We are seeking a highly skilled and compassionate Registered Nurse to join our team as a Weekend Home Health RN. As a key member of our healthcare team, you will provide high-quality patient care in the comfort of their own homes.

About the Role:
  • Provide comprehensive nursing care to patients in their homes, as prescribed by the physician and in compliance with applicable laws and regulations.
  • Coordinate total plan of care with other healthcare professionals involved in care and help achieve and maintain continuity of patient care.
  • Make initial and/or comprehensive nursing evaluation visits, determine primary focus of care, develop the plan of care within state-specific guidelines, and submit accurate, complete, and timely documentation in collaboration with physician orders.
  • Ensure patients meet home health eligibility and medical necessity guidelines as defined by payer source.
Requirements:
  • Current RN licensure in the state of practice.
  • Current CPR certification.
  • Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
