Job Summary

A qualified Licensed Practical Nurse administers skilled nursing care to patients in their place of residence, following physician orders and a plan of care under the direction of a Registered Nurse.

Key Responsibilities
  • Implement the nursing care plan for each patient, providing nursing services, treatments, and diagnostic and preventive procedures as assigned.
  • Initiate preventive and rehabilitative nursing procedures as necessary, preparing equipment and materials for treatments.
  • Assist the RN in performing specialized procedures, observing aseptic technique as required.
  • Evaluate the effectiveness of the LPN's nursing service to the patient and family under the guidance of the registered nurse.
  • Maintain accurate and complete records of observations, treatments, and care of the patient.
Teaching and Counseling

Teach and counsel the patient and family/significant others regarding the nursing care needs and other related problems of the patient at home, assisting the patient in learning appropriate self-care techniques.

Collaboration and Communication

Participate in patient care conferences, discussing the need for involvement of other members of the health team, such as physical therapist, speech therapist, occupational therapist, and medical social worker.

Administrative Tasks

Submit any changes in schedule to the clinical manager on a daily basis, understand and adhere to established policies and procedures, and participate in medical record audits as assigned.

This is a non-management job that will report to a supervisor, manager, director, or executive.

Requirements
  • Completion of a LPN program recognized by the SC State Board of Nursing.
  • One year experience as a LPN.
  • Experience in Home Health preferred.
Certifications/Registrations/Licenses
  • Licensed to practice as a LPN in South Carolina.
  • SC Driver's license.

Driving is required for this position using the employee's personally owned vehicle, and the employee must have reliable transportation and adhere to the Prisma Health Driver Safety Policy and specific department driving policies.
