Join Our Team as a Physical Therapist

We are seeking a highly skilled and experienced Physical Therapist to join our team at Centra Healthcare Solutions. As a Physical Therapist, you will be responsible for providing high-quality patient care in a skilled nursing facility (SNF) or long-term care (LTC) setting.

Responsibilities:
  • Provide physical therapy services to patients with various medical conditions, including those with mobility and functional impairments.
  • Develop and implement individualized treatment plans to improve patient outcomes and quality of life.
  • Collaborate with healthcare teams to ensure comprehensive patient care and coordinate services.
  • Conduct thorough assessments and evaluations to identify patient needs and goals.
  • Communicate effectively with patients, families, and healthcare professionals to ensure seamless care transitions.
Requirements:
  • Current Physical Therapist license in the state of practice.
  • 1 year of recent experience as a Physical Therapist in a SNF or LTC setting.
  • Strong clinical skills and ability to work independently.
  • Excellent communication and interpersonal skills.
