El rol del Enfermero Coordinador de Cuidados es garantizar que cada paciente reciba el nivel de atención adecuado y que la transición al siguiente nivel sea apropiada y completa.

El Enfermero Coordinador trabajará bajo la dirección del departamento para coordinar un enfoque multidisciplinario que organice y coordine una atención efectiva, eficiente y de calidad.

El coordinador colaborará con el equipo multidisciplinario para desarrollar un plan de transición que sea adecuado para el paciente en relación con los beneficios, recursos y condiciones médicas.

El coordinador utiliza activamente la evaluación, planificación, implementación, coordinación, monitoreo y evaluación. El coordinador se esfuerza por mejorar la calidad y la eficiencia de costos de la atención médica y la calidad de vida del cliente.

Educación
Título de Asociado o 2 años de cursos universitarios

Experiencia
Dos años de experiencia relacionada. Se requiere conocimiento práctico de prácticas, equipos y procedimientos especializados.

Licencias y Certificaciones
*LICENCIA DE ENFERMERO REGISTRADO DE GEORGIA O LICENCIA DE ENFERMERÍA MULTIESTATAL

Cualificaciones
*Se requiere un mínimo de dos (2) años de experiencia clínica, preferiblemente en medicina quirúrgica.
*Se prefiere experiencia en gestión de casos.
*Capacidad para funcionar dentro de un concepto de equipo sinérgico.
*Licencia actual de enfermero en Georgia. Se prefiere BSN/MSN.
*Capacidad demostrada para analizar y organizar información y datos.
*Capacidad demostrada para organizar, priorizar y tomar decisiones apropiadas siguiendo las políticas y procedimientos organizacionales y departamentales con mínima supervisión.
*Iniciativa demostrada, pensamiento crítico y habilidades para resolver problemas.
*Habilidades interpersonales demostradas. Excelentes habilidades de comunicación verbal y escrita.
*Profesionalismo, integridad y comportamiento ético demostrados.
*Conocimiento de pautas de revisión de utilización, principios de gestión de casos y estándares regulatorios y de acreditación.
Descripción del Turno
Lunes a Viernes de 8 a 4:30 o de 8 a 5:30pm, rotación ocasional de fin de semana cada 8-10 semanas.
