Job Summary

We are seeking a highly skilled Physical Therapist to join our team at Acts Retirement-Life Communities. As a Physical Therapist, you will be responsible for providing physical therapy services to residents, including evaluations, planning individual treatment programs and goals, actual treatments, discharge planning, and performing other related duties as required.

Key Responsibilities
  • Conduct thorough evaluations of residents to identify physical therapy needs and develop individualized treatment plans
  • Provide physical therapy services, including exercises, modalities, and other interventions, to residents in skilled nursing and outpatient settings
  • Collaborate with other healthcare professionals to ensure comprehensive care and effective treatment outcomes
  • Document resident progress and treatment plans in accordance with company policies and regulatory requirements
  • Participate in ongoing education and training to stay current with best practices and industry developments
Requirements
  • Bachelor's degree in Physical Therapy
  • Valid Physical Therapist license or eligibility for license in the state(s) where services are rendered
  • Experience in a skilled nursing and/or outpatient rehab environment is preferred
