Job Summary:
This position provides comprehensive nutrition services and education to dialysis patients and authorized caregivers in a clinical setting to support and maximize the patient's nutritional well-being within a chronic-care facility for end-stage renal disease patients in accordance with LifeBridge Health policies, procedures, and guidelines, OSHA, CMS, federal, state, and local regulations.
- Nutrition Assessment and Intervention: Assess nutritional status to determine appropriate nutrition intervention and recommendations.
- Interdisciplinary Care Plan: Participate in an interdisciplinary care plan based on assessment or re-assessment with 15 days after completion of assessment, annually for stable patients and monthly for unstable patients.
- Nutrition Education and Counseling: Provide nutrition education and counseling based on individualized patient needs, including nutrient requirements, knowledge, and abilities, medical, economic, and social situation.
- Monitoring and Documentation: Monitor nutritional status, laboratory values, dialysis adequacy, adherence to diet, and response to nutrition-related medications and therapy to determine appropriate nutrition intervention and recommendations.
- Quality Patient Care: Administer quality patient care in a considerate, respectful manner always.
Requirements:
- Education: Bachelor's degree in nutrition, dietetics, or a similar area required.
- Registration: Registered Dietitian status with the Commission on Dietetic Registration required.
- Licensure: State licensure required in the state of Maryland or where the facility is located.
- Experience: Minimum of two years' professional work experience in clinical nutrition as a Registered Dietitian required.
