Vivo HealthStaff is seeking an experienced Physician Assistant or Nurse Practitioner to join our team as a Primary Care Provider. This is a permanent full-time position with benefits.
We are looking for a skilled professional who is passionate about delivering high-quality patient care in a fast-paced and dynamic environment. As a Primary Care Provider, you will be responsible for providing onsite primary care services to older adults living in Senior Living Communities.
Key Responsibilities:
- Develop and execute detailed person-centered care plans for all primary care patients
- Work with Medical Director to identify and develop policies to address issues that arise in day-to-day practice, culture, or clinical guidelines
- Collaborate with Care Navigators to provide monthly Chronic Care Management (CCM) visits and care, reviewing and signing off on CCM documentation
- Run regular huddles and provide leadership to the clinical team
- Identify ways to work with senior living management and care teams to better carry out Pine Park care plans and improve patient outcomes
Requirements:
- Family Medicine or Nurse Practitioner or Physician Assistant experience, preferably with geriatrics experience
- Comfortable working in a fast-paced and changing environment
- Willingness to provide onsite clinical care across the San Diego market
- Desire to work with complex care patients and support the building of a holistic care model
- Strong orientation towards collaboration and working closely with a multidisciplinary team
- Strong written and verbal communication skills
- Ability to work with tech-enabled care delivery tools to improve patient care and team function
