About the Role

Mercy Home Health is seeking a skilled and compassionate Registered Nurse to join our team as a Home Health RN. As a Home Health RN, you will provide high-quality, patient-centered care to individuals in their homes, promoting independence and well-being.

Responsibilities
  • Assess, plan, implement, document, evaluate, and teach patients' needs and care.
  • Provide skilled nursing services to patients in their homes under the direction of a physician.
  • Manage a caseload of patients experiencing a wide range of health conditions.
  • Communicate effectively with patients, families, and healthcare professionals to ensure seamless care coordination.
  • Document patient information accurately and thoroughly, including changes to medication and plan of care.
Requirements
  • Current RN license in the state of Missouri.
  • One year of experience in a related field preferred.
  • Graduation from an accredited nursing school.
  • BLS CPR Certification through the American Heart Association.
