About Us: The Louisiana Primary Care Association (LPCA) is dedicated to ensuring that quality primary healthcare services are accessible and affordable for uninsured and underserved communities in Louisiana. As a membership organization comprising Federally Qualified Health Centers (FQHCs) and various supporters, LPCA is committed to achieving universal healthcare access.

Key Responsibilities:

  • Enhance access to health services for targeted populations in collaboration with the Care Management Team.
  • Develop and disseminate complex analytical models to improve data literacy among stakeholders.
  • Lead initiatives focused on practice transformation and performance enhancement.
  • Support Health Centers in meeting specific objectives to boost quality, efficiency, and service utilization.
  • Work alongside care managers, health plans, and data analytics teams to pinpoint areas for improvement.
  • Utilize data to generate actionable insights for stakeholders, identifying trends in prescribing and potential improvement opportunities.
  • Collaborate with the Care Management Team to enhance chronic disease management and promote preventive care, thereby reducing unnecessary healthcare utilization.
  • Create a transformation strategy aimed at improving utilization metrics and quality measures based on performance data.
  • Facilitate productive practice improvement meetings to track and discuss the progress of transformation initiatives.
  • Analyze cost, utilization, and quality data from diverse sources, including payer portals and claims.
  • Establish and maintain effective communication with key stakeholders.
  • Leverage data dashboards to educate and promote changes in clinical workflows.
  • Review monthly reports from practice teams and implement improvement strategies.
  • Collaborate with the Care Management Team to evaluate project advancements.
  • Perform additional duties as assigned.

Qualifications:

  • Bachelor's degree in healthcare or a related field, or equivalent experience in the healthcare sector.
  • Preferred experience or coursework in quality improvement.
  • Proficiency in Microsoft Excel, Word, PowerPoint, Outlook, and Zoom.
  • Familiarity with Electronic Medical Records (EMR) is advantageous.
  • Ability to perform physical tasks such as sitting, standing, and moving independently.
  • Effective communication skills are essential.
  • Must possess reliable transportation and maintain a valid driver's license.
