Job Summary:

At Falcon Healthcare, we are seeking a skilled Nurse Care Coordinator to join our team. As a key member of our home health care team, you will be responsible for providing high-quality patient care and services to our in-home clients.

Key Responsibilities:

• Develop and implement individualized care plans to meet the unique needs of each patient
• Conduct thorough assessments of patients' physical and emotional needs
• Collaborate with healthcare professionals to ensure seamless care coordination
• Provide education and support to patients and their families to promote healthy living and independence

Requirements:

• Current RN License – active and in good standing
• Minimum of 1-year nursing experience preferred
• Home Health experience is a plus
• Current CPR certification
• Strong organizational and communication skills
• Proof of current auto insurance
• Proof of Covid vaccine, medical exemption or religious exemption
