Essential Qualifications:

Key Responsibilities:
  • Provide nursing and supportive care in accordance with the Interdisciplinary Plan of Care, under the guidance of the RN
  • Work collaboratively with attending physicians, home health team members, and other organizations to ensure optimal care and effectively utilize community resources for the patient and family.
  • Maintain documentation of all services provided.
  • Demonstrate knowledge of Medicare/Medicaid regulations related to home health benefits.
  • Communicate any changes in patient condition and needs to the RN.
