At Interim HealthCare of New Hampshire, we're dedicated to delivering personalized, home-based therapy that empowers patients to regain their strength, manage their pain, and reach their goals. As a Home Health Physical Therapist Assistant, you'll play a vital role in our team of healthcare professionals, working closely with patients and their families to create a tailored plan of care that addresses their unique needs.

Key Responsibilities:
  • Provide home-based physical therapy to patients who have lost mobility, strength, and functionality due to illness or injury
  • Work collaboratively with a multidisciplinary team, including RNs, LPNs, CNAs, HHAs, OTs, and SLPs, to develop and implement patient care plans
  • Assist patients with exercises and hands-on therapy to manage pain and increase mobility
  • Educate patients and their families on therapy goals, exercises, and self-care
  • Coach family members on safely assisting with exercises and promoting patient independence
Requirements:
  • Active PTA license in Maine
  • Minimum of 1 year of PTA experience, ideally in home healthcare
  • CPR certification
  • Up-to-date on all State of Maine required vaccinations
  • Knowledge of state and federal home health regulations
  • Good clinical judgment, strong interpersonal skills, dexterity, and compassion
