**Job Overview**

This position plays a crucial role in providing continuity of care, with the goal of permanent placement, for children through a family care management model. This involves developing, expanding, accessing, and linking resources in the community to meet the needs of the child throughout their experience in the system, while documenting progress.


**Key Responsibilities:**
  • Assess, interpret, and provide care information for children and families.
  • Work as part of a Family Care Team managing children (ages 0-17).
  • Develop and maintain service/case plans with clear goals, objectives, and timelines.
  • Ensure completion of Medicaid assessments and case/treatment plans for children in foster care.

The ideal candidate will have strong verbal and written communication skills, as well as the ability to document clearly and concisely. A positive attitude, patience, respect, and cultural sensitivity when dealing with challenging environments are essential qualities for this role.


**What We Offer:**
  • A competitive salary of $53,000 - $65,000 per year, based on experience.
  • An estimated salary range of $55,000 - $70,000 per year, considering location and other factors.
