Job Summary:

The Anticoagulation Pharmacist - Casual Position is responsible for managing patients on warfarin, ensuring their safety and effectiveness through regular monitoring and dosing adjustments.

Key Responsibilities:

  • Review prescriptions for safety and accuracy, checking patient allergies, weight, renal function, and lab test results.
  • Make recommendations for alternative medications if necessary.
  • Attend interdisciplinary meetings to provide information and serve as a pharmacy resource.
  • Provide exceptional service to patients and nurses filling orders.

Qualifications:

  • Pharmacist License
  • Exceptional service skills

Pay Range: $67.77/hour
