Job Title: Physical Therapist III, Home Health

We are seeking a highly skilled Physical Therapist III to join our Home Health team at Sutter Health. As a key member of our interdisciplinary team, you will be responsible for providing high-quality patient care and management in the home setting.

Job Summary:

The Physical Therapist III will work under the supervision of the Clinical Manager to provide direct care to patients in their homes. This includes assessing, planning, organizing, and participating in rehabilitative programs to improve patient mobility, relieve pain, and increase strength. The ideal candidate will have advanced knowledge and practice level competencies, as well as excellent communication and interpersonal skills.

Responsibilities:
  • Assess patients' physical and functional abilities to develop individualized treatment plans
  • Implement and evaluate the effectiveness of treatment plans, making adjustments as needed
  • Collaborate with other healthcare professionals to ensure comprehensive patient care
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination
  • Participate in agency-wide orientation, competency evaluations, and consultation to clinical staff as requested by the Clinical Manager
Requirements:
  • Graduate of an accredited physical therapy school
  • California Physical Therapist License
  • BLS-Basic Life Support Healthcare Provider certification
  • DL-California Drivers Class C
  • Automobile Insurance
Preferred Experience:

1 year of experience in primary work assignment area

Skills and Knowledge:
  • Critical thinking and complex problem-solving skills
  • Ability to work independently and as part of a team
  • Knowledge of professional practice and regulatory requirements
  • Effective communication and interpersonal skills
  • Ability to analyze and identify clinical practice and workflow best practices
Work Environment:

This is a full-time position with a rotating weekend schedule. The Physical Therapist III will work in a fast-paced home health environment, requiring the ability to prioritize tasks, manage multiple priorities, and meet deadlines.

Sutter Health offers a comprehensive benefits package, including medical, dental, and vision insurance, 401(k) matching, and paid time off.

Pay Range:

$68.10 to $85.11 per hour

This range has not been adjusted for any specific geographic differential applicable by area where the position may be filled.

Compensation takes into account several factors, including but not limited to, a candidate's experience, education, skills, licensure and certifications, department equity, training, and organizational needs.
