We are seeking a skilled Licensed Practical Nurse to join our interdisciplinary team at Lifespace Communities. As a key member of our care coordination team, you will play a vital role in delivering high-quality care to our residents.

Responsibilities
  • Participate in the development and implementation of individualized care plans for residents, in collaboration with the interdisciplinary team.
  • Document resident conditions and nursing needs accurately and in a timely manner, ensuring that all relevant information is communicated to the healthcare team.
  • Coordinate and facilitate communication with families regarding changes in medications and/or care plans.
  • Develop and implement protocols to ensure that care plans are continually reviewed and updated to reflect the individual needs of each resident.
  • Coordinate admissions, discharges, and transfers to ensure seamless transitions and high-quality customer service.
Requirements
  • Graduate of an accredited Licensed Practical Nurse program.
  • Current licensure as a Licensed Practical Nurse in good standing.
  • Ability to work collaboratively as part of an interdisciplinary team.
  • Excellent communication and documentation skills.
