Job Summary

Key Responsibilities:
  • Conduct thorough assessments of patients and their families to identify their needs and develop individualized care plans.
  • Collaborate with healthcare professionals to coordinate patient care and ensure seamless transitions between departments.
  • Provide education and support to patients and their families on health-related topics and disease management.
  • Develop and implement treatment plans that address the physical, emotional, and social needs of patients.
  • Monitor patient progress and adjust care plans as needed to ensure optimal outcomes.
Requirements:
  • Associate degree (ADN) from an ACEN or CCNE accredited school of nursing or diploma in nursing required.
  • Bachelor's degree (BSN) from ACEN or CCNE accredited school of nursing desired.
  • Dementia/Psychiatric experience preferred.
  • Current Registered Nurse licensure in the state of Kansas.
  • Certification in CPR/Basic Life Support.
Preferred Qualifications:
  • Experience working in a Senior Behavioral Health Center or similar setting.
  • Knowledge of evidence-based practices and treatment modalities for dementia and psychiatric patients.
  • Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare professionals.
