Key Responsibilities:- Conduct thorough assessments of patients and their families to identify their needs and develop individualized care plans.
- Collaborate with healthcare professionals to coordinate patient care and ensure seamless transitions between departments.
- Provide education and support to patients and their families on health-related topics and disease management.
- Develop and implement treatment plans that address the physical, emotional, and social needs of patients.
- Monitor patient progress and adjust care plans as needed to ensure optimal outcomes.
- Associate degree (ADN) from an ACEN or CCNE accredited school of nursing or diploma in nursing required.
- Bachelor's degree (BSN) from ACEN or CCNE accredited school of nursing desired.
- Dementia/Psychiatric experience preferred.
- Current Registered Nurse licensure in the state of Kansas.
- Certification in CPR/Basic Life Support.
- Experience working in a Senior Behavioral Health Center or similar setting.
- Knowledge of evidence-based practices and treatment modalities for dementia and psychiatric patients.
- Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare professionals.
