Assess the patient's readiness for discharge and coordinate with the healthcare team to ensure a smooth transition to post-acute care.

Provide Education and Support

Provide education and support to patients and their families, promoting health literacy and empowering them to take an active role in their care.

Requirements
  • Education and Certification

Graduate from a fully accredited RN program and hold an active, unencumbered RN license in the state of Pennsylvania.

Experience

Minimum of two years of experience as a Registered Nurse in a telemetry or emergency department setting.

Skills and Abilities

Strong technical skills, including proficiency in electronic medical records and other healthcare software. Excellent communication and interpersonal skills, with the ability to work effectively in a fast-paced environment.

Preferred Qualifications
  • Basic Life Support (BLS) Certification

Current BLS certification, with the ability to obtain ACLS certification within six months of hire.

Advanced Cardiovascular Life Support (ACLS)

Current ACLS certification, with the ability to maintain certification through ongoing education and training.
