Job Summary

National Care Advisors is seeking a highly skilled Nurse Case Manager to join our team in the Bay Area. As a key member of our team, you will be responsible for providing case management services to special needs and eldercare clients, ensuring their quality of life and collaborating with primary family caregivers, attorneys, financial planners, and trustees.

Key Responsibilities
  • Develop and implement individualized care plans to meet the unique needs of clients
  • Collaborate with healthcare providers, family members, and other stakeholders to ensure seamless care coordination
  • Conduct thorough assessments of clients' needs and develop strategies to address challenges
  • Provide education and support to clients and their families on healthcare options and resources
  • Manage and coordinate third-party benefits, including health insurance, Medicaid, Social Security, and Developmental Disability services
  • Maintain accurate and timely documentation of client interactions and care plans
  • Participate in regular team meetings and contribute to the development of best practices in case management
Requirements
  • Bachelor of Science in Nursing (BSN) required; Certified Case Manager (CCM) or Certified Rehabilitation Registered Nurse (CRRN) preferred
  • Minimum of 5 years clinical nursing experience, with a focus on field case management or workers' compensation case management
  • Excellent communication and interpersonal skills, with the ability to work effectively with diverse stakeholders
  • Proficient in using MacOS and a variety of related software applications, including MS Office
  • Strong ability to quickly learn and adapt to new technologies and tools in a Mac environment
  • Highly motivated self-starter with solid time management and organization skills
  • Proven ability to collaborate as necessary to accomplish goals and work through conflicts
  • Ability to research and develop solutions to challenges presented by clients
  • Excellent customer service skills
  • Willingness to meet timely documentation requirements
