Job Summary

Kaiser Permanente is seeking a skilled Registered Nurse Navigator to join our team. As a key member of our healthcare team, you will play a vital role in providing exceptional patient care and navigation services.

Key Responsibilities
  • Assist patients and families in understanding their diagnosis, treatment options, and available resources within Kaiser Permanente.
  • Develop and facilitate individualized plans of care in consultation with treating physicians, patients, and their support networks.
  • Serve as a liaison between patients, healthcare providers, family, and support teams, ensuring seamless coordination of care.
  • Perform care assessments, triage new and ongoing patients, and facilitate appointments for consults, procedures, and support services.
  • Deliver patient and family education, maintain a resource library, and standardize patient education materials.
  • Monitor patient compliance with treatment plans, assist with referrals to outside resources, and coordinate care as needed.
  • Develop and maintain high-quality evidence-based care processes through patient education and training.
  • Foster relationships with Kaiser Enterprise and community resources to support patient care.
  • Participate in quality improvement initiatives, research projects, and strategic planning processes.
Requirements
  • Minimum three (3) years of current RN clinical experience in the area of specialty.
  • Associates degree in nursing.
  • Registered Professional Nurse License (Georgia) required at hire.
  • Basic Life Support required at hire from American Heart Association.
  • Advanced Cardiac Life Support (ACLS) Required at hire (In Cardiac specialties ONLY).
  • Specialty Nursing Certification in relevant field within 12 months of hire.
