Alternate Solutions Health Network LLC is seeking a skilled Occupational Therapist to join our Home Health team in North Kansas City, Missouri.

Job Summary:

We are looking for a compassionate and detail-oriented Occupational Therapist to provide high-quality care to our patients in the comfort of their own homes. As an Occupational Therapist with our team, you will work closely with patients, families, and healthcare professionals to develop and implement individualized plans of care that promote independence, mobility, and overall well-being.

Responsibilities:
  • Conduct thorough assessments of patients' physical and cognitive abilities to identify areas of need and develop effective treatment plans.
  • Provide occupational therapy services to patients in their homes, including exercises, activities, and education to promote independence and mobility.
  • Collaborate with patients, families, and healthcare professionals to ensure seamless care and communication.
  • Document patient progress and outcomes in a timely and accurate manner.
  • Participate in ongoing education and training to stay current with best practices and industry developments.
Requirements:
  • Current licensure as an Occupational Therapist in the state of Missouri.
  • Minimum one year of experience as an Occupational Therapist in a home health setting.
  • Valid driver's license and reliable transportation.
  • Ability to work independently and as part of a team.
