Healthcare Case Management Specialist (Full-Time, Permanent)
N2S Healthcare Staffing Solutions LLC is in search of a committed Healthcare Case Management Specialist who will be accountable for the strategic planning, organization, and coordination of patient admissions, particularly in swing bed scenarios, alongside evaluating emergency department admissions and discharges. The successful candidate will possess a robust background in clinical nursing and case management, demonstrating the capability to deliver both administrative and clerical support. This position emphasizes process enhancement, management of medical documentation, and collaboration with referral specialists.
Key Responsibilities:
- Conduct financial evaluations to ascertain patients' payment options for inpatient services.
- Respond to inquiries from medical offices, families, and external facilities with accurate information.
- Administer case management, social services, utilization review, and discharge planning on a daily basis, including weekends as required.
- Identify and facilitate swing bed admissions, ensuring compliance with admission prerequisites, including necessary screenings and assessments.
- Review admissions and discharges from the emergency department, reaching out to patients to arrange follow-up appointments or transfers as needed.
- Establish and nurture relationships with specialty clinics to support care transitions or collaborations.
- Engage in interdisciplinary team discussions and participate in Utilization Review (UR) meetings.
- Examine patient records for counseling requests and evaluate the necessity for intervention or community resources.
- Availability for at least two Saturdays per month, contingent on admissions.
Skills and Abilities:
- Bilingual in Spanish is preferred.
- Exceptional multitasking abilities and proficiency in a dynamic work environment.
- Familiarity with Microsoft Office, Athena, EMR, and Point Click Care.
- Strong analytical skills aimed at enhancing patient care.
- Capability to educate patients and their families regarding health conditions and treatment options.
- Ability to collaborate effectively with ancillary departments on patient care strategies.
- Awareness of community resources and support services available.
Qualifications:
- Three to five years of experience in clinical nursing.
- One to two years of experience in case management or a related discipline is preferred.
- Current state licensure as a Registered Nurse (RN).
- Bachelor's degree in nursing is highly preferred.
- BLS certification is mandatory; ACLS certification is preferred.
