LPN Medical Assistant

We are seeking a skilled Licensed Practical Nurse (LPN) to join our team at Riverside Health System in Hampton, Virginia. As a key member of our Rheumatology department, you will play a vital role in providing high-quality patient care and contributing to the success of our medical practice.
Key Responsibilities- Conduct focused assessments of patients, including vital signs and data gathering, to inform care plans.
- Implement established plans of care and evaluate the effectiveness of treatments in a medical practice setting.
- Accurately complete all required clinical documentation, maintaining quality assurance standards.
- Work collaboratively with providers to address care gaps and ensure accurate, legible documentation.
- Provide patient education and teach patients about their medications, promoting positive patient care.
- Communicate lab/pathology results, schedule additional tests, and ensure proper reimbursement from insurance companies.
- Demonstrate a commitment to building trusting relationships with a multi-disciplinary team to support patient care needs.
- Program Graduate, Licensed Practical Nurse (Preferred)
- Bachelors Degree, Nursing (Preferred)
- 1 year Nursing experience (Preferred)
- Licensed Practical Nurse (LPN) - Virginia Department of Health Professions (VDHP) Upon Hire (Required)
- CPR/BLS Certification - American Heart Association/American Red Cross/American Safety and Health Institute (AHA/ARC) Upon Hire (Required)
