Family Nurse Practitioner

We are seeking a highly skilled and compassionate Family Nurse Practitioner to join our team at School Health Clinics of Santa Clara County. As a key member of our healthcare team, you will provide high-quality, patient-centered care to children and adolescents in a dynamic and supportive environment.
Key Responsibilities- Provide comprehensive, direct medical care to patients, including reproductive health services, under the direction of a physician and using established protocols.
- Teach individuals and groups about health-related topics, promoting healthy behaviors and disease prevention.
- Perform or oversee the performance of accurate laboratory tests, such as urine dipsticks, pregnancy tests, and hemoglobin checks, and follow up on results.
- Dispense pre-packaged, pre-labeled medications under established protocols.
- Maintain accurate, problem-oriented medical records for each patient.
- Serve as a liaison with school staff, including teachers, counselors, and administrators, to ensure seamless care coordination.
- Provide direction and supervision to assigned Medical Assistants.
- Assist in data collection for evaluation purposes, completing Encounter Forms for each patient.
- Complete necessary billing forms for each patient.
- Ensure the smooth operation of the assigned clinic, overseeing day-to-day activities and maintaining a well-organized environment.
- Certification or licensure as a Physician Assistant or Family Nurse Practitioner in California.
- Graduate of a NCCPA or FNP program.
- Bachelor's degree in a related field.
- Knowledge of general and specific medical diagnoses and treatments, especially for children and adolescents.
- Problem-oriented recording and pharmaceutical treatments.
- Health education methods and techniques, and referral resources within the community.
- Primary, acute, and chronic healthcare management, and basic laboratory work.
Usual clinic environment with frequent interruptions in work to answer telephones and deal with crises. May be exposed to communicable diseases.
Reporting RelationshipsUnder the general administrative supervision and general clinical supervision of the Medical Director.
