Vivo HealthStaff seeks a highly skilled Clinical Care Manager to provide exceptional care to patients in Kentucky. This 100% remote position requires a Licensed Clinical Social Worker (LCSW) with experience in addiction treatment.

As a Clinical Care Manager, you will work closely with an addiction medicine provider to manage a panel of patients with varying levels of need. Your responsibilities will include:

  • Coordinating care between patients, healthcare providers, and specialty mental health services.
  • Documenting patient encounters and providing timely updates to the multidisciplinary team.
  • Researching and facilitating referrals to community services and additional treatment programs based on patient need.

You will also provide direct services to patients, including brief individual and group-based treatment using evidence-based interventions such as motivational interviewing, behavioral activation, problem-solving treatment, and CBT. Additionally, you will develop and implement group programming for patients with co-occurring disorders.
