Job Summary

Public Partnerships LLC is seeking a Support Broker to work with individuals with disabilities or chronic illnesses and aging adults to remain in their homes and communities. As a Support Broker, you will provide information and education to program participants and/or their authorized representatives, evaluating their home environment and resolving safety concerns through our mandated escalations process.

Key Responsibilities
  • Visit participants in their homes, providing information and education on self-direction and home care services.
  • Evaluate home environments, identifying and resolving safety concerns.
  • Assist participants in managing their home care services, ensuring efficient use of resources.
  • Collaborate with team members to provide comprehensive support to participants.
Requirements
  • BA or BS degree in social work, case worker, sociology, psychology, LPN, or related field.
  • 1 year experience working with vulnerable populations in their homes.
  • Experience with Microsoft Office products, including Outlook.
  • Familiarity with CRM tools and electronic medical records.
Working Conditions
  • Visiting participants in their homes, which may involve climbing stairs.
  • Significant amount of driving, potentially exceeding 60 miles each way.
  • Administrative work at home, including typing, phone calls, and Teams meetings.
