About the RoleAs a Home-Based Medical Provider, you will be responsible for providing primary, urgent, and acute medical care to patients in the comfort of their own homes. You will work collaboratively with our interdisciplinary team to develop and implement personalized care plans that address the unique needs of each patient.
Responsibilities- Provide comprehensive medical care to patients in their homes, including routine check-ups, urgent care, and acute medical interventions.
- Collaborate with our care community to develop and implement patient-centered care plans that address physical, emotional, and social needs.
- Facilitate staff, patient, and family decision-making through education and support.
- Address urgent care needs of patients as needed, including emergency situations.
- Deliver quality care while maintaining all company metrics pertaining to documentation, coding, HEDIS measures, patient engagement, and medical expense management.
- Master's Degree in Nursing or related field.
- Valid nurse practitioner or physician assistant license in the state of operations and current board certification in your field.
- Valid driver's license, car insurance, and access to a car for patient visits.
- Expertise in diagnosis and treatment of chronic and urgent medical/psychological conditions of older adults.
- Experience in end-of-life, geriatrics, internal medicine, and palliative care.
- Strong computer skills, including Word, Excel, and PowerPoint.
