HireLiving, Inc. is actively seeking Care Coordinators who will collaborate with emergency care staff to assist youth experiencing behavioral health crises. This role involves connecting youth and their families to essential community resources and therapeutic services tailored to their needs.
Care Coordinators play a vital role in a program initiative aimed at providing intensive service coordination and family support resources, ultimately enhancing family well-being by linking them to community resources and fostering self-advocacy.
Under general supervision, Care Coordinators will deliver a wide range of social services to clients and their families. Responsibilities include conducting client intakes, participating in treatment planning, and performing crisis intervention. They will assist clients in navigating personal and social challenges and may provide supportive counseling or serve as a liaison for social services. Case management duties may also be performed.Duties & Responsibilities:
- Support clients and their families during emergency visits. Collect relevant demographic and clinical information for client profiles. Conduct interviews with clients and family members to assess needs and gather necessary information for service identification. Provide information and referrals to local services and guide families through intake procedures. Follow up with clients to ensure successful connections to services.
- Establish and maintain a network of community contacts. Facilitate referrals and linkages to community services to ensure a continuum of care. Act as a liaison to local human services agencies, community resources, and insurers. Collaborate with agencies and service providers involved with youth and families to streamline connections and referrals.
- Document observations and interactions with clients accurately and concisely in their records, ensuring timely updates.
- Notify supervisory staff of any unusual incidents or observations. Participate in team meetings to provide input on workflow, issues, successes, and overall program enhancement.
- Engage positively and respectfully with clients and families.
Minimum Qualifications:
A bachelor's degree in a social service-related field from an accredited institution AND one year of experience in a related field OR a high school diploma or GED AND three years of experience in a social services role OR one year of experience at the lower level Social Services Tech 2 (SST011) or equivalent position.
Preferred Qualifications:
- Bachelor's degree in a related field and two years of professional experience in human services. Knowledge of community resources.
- Experience in case management and crisis intervention.
- Ability to engage with a diverse range of clients in an emergency setting, demonstrating flexibility in response to varying needs.
- Energetic, compassionate, and resourceful.
- Awareness of issues related to homelessness, mental health, substance use, poverty, and social marginalization.
- Strong oral, written, and technical communication skills.
- Exceptional organizational and time management abilities, with creative problem-solving skills.
- Ability to work independently and collaboratively within a multidisciplinary team.
- Knowledge of resources available across Georgia for individuals served.
- Understanding of relevant policies and procedures.
Requirements/Competencies:
- Must be able to lift 20 pounds.
- Requires prolonged periods of sitting and standing.
- Must be at least 21 years old.
- Must possess a valid driver's license and maintain a satisfactory Motor Vehicle Record.
- Candidates must pass a criminal background check.
- Must pass a pre-employment drug screening and be subject to random drug tests.
- Any combination of training and experience that would enable the applicant to acquire the necessary knowledge, skills, and abilities.
Note:
Some positions may require a valid driver's license.
