Position Summary:

The Palliative Care Registered Nurse will report directly to the Director of Palliative Care. In this role, you will engage with patients, their families, and other healthcare professionals to deliver and coordinate high-quality end-of-life care. The Palliative Care RN will conduct both in-person and telephonic assessments for palliative care patients in collaboration with the interdisciplinary team. The primary responsibility includes performing in-person assessments in various settings such as private residences, assisted living facilities, and skilled nursing homes. You will also provide education to patients and their families, ensuring comfort within their home environment.

Key Responsibilities:

  • Conduct in-person and telephonic assessments and provide education to palliative care patients and their families.
  • Complete Risk Stratification Assessments to identify patient support needs in consultation with the Nurse Practitioner.
  • Monitor patient visit frequency based on individual needs and risk assessments.
  • Deliver reliable education and instructions to patients and families, ensuring excellent customer service.
  • Consult with patients and families regarding disease processes, self-care techniques, end-of-life care, medications, and nutritional needs.
  • Communicate findings that indicate the need for preventive and rehabilitative skilled nursing or therapy to the Primary Care Provider or Palliative Nurse Practitioner.
  • Educate patients and families on medication administration and treatments as outlined in the care plan.
  • Coordinate care with members of the palliative care team.
  • Report changes in patient condition or signs of decline to the Palliative Nurse Practitioner and/or Primary Care Provider.
  • Facilitate hospice referrals as necessary.
  • Document clinical records promptly, including assessments, physician orders, progress notes, and care plans.
  • Support the development and implementation of care plans for palliative care from initial assessment through discharge.
  • Collaborate on the development and adjustments of individualized palliative care plans in consultation with patients, families, and team members.
  • Assist in obtaining necessary care for patients when needs are identified according to the care plan.
  • Participate in on-call services as required, demonstrating clinical competence and responsiveness.
  • Facilitate prescription transmission to pharmacies, including controlled substances, as permitted by state law.
  • Communicate clinical findings to the Nurse Practitioner and/or Primary Care Provider regularly.
  • Maintain effective working relationships with the supportive care team and involve them when necessary to achieve desired outcomes.
  • Document clinical findings and care provided in the patient's medical record in accordance with accepted standards.

Qualifications:

Education:

  • Registered Nurse (RN)

Experience:

  • Minimum of 2 years of experience as a Registered Nurse.
  • Proven ability to deliver quality patient care.
  • Preferred experience in palliative care, oncology, hospice, or geriatric care.
  • Additional experience in hospital settings, emergency care, critical care, or similar fields is advantageous.

Licenses and Certifications:

  • Current RN license to practice.
  • CPR Certification.

Skills:

  • Proficient in Microsoft Office Suite and electronic medical record documentation.
  • Strong communication skills, both written and verbal.
  • Ability to maintain patient confidentiality and demonstrate good problem-solving and time management skills.
