Job Summary

Key Responsibilities
  • Deliver Direct Patient Care
    • Administer medications and treatments as prescribed by physicians and RNs
    • Monitor and record patients' vital signs, including blood pressure, pulse, temperature, and respiration rates
    • Provide wound care, including cleaning, dressing, and managing wounds, incisions, and injuries
  • Conduct Comprehensive Patient Assessments
    • Perform physical exams and medical histories to craft personalized care plans
    • Review medication lists and ensure accurate dosing and adherence to established protocols
  • Collaborate with Healthcare Team
    • Work closely with RNs, physicians, therapists, and support staff to ensure seamless patient care
    • Communicate effectively with patients and families to address their needs and concerns
  • Maintain Accurate Patient Records
    • Document changes in patients' conditions, administered treatments, and medication management
    • Ensure accurate and timely documentation of patient information
  • Provide Education and Support
    • Educate patients and families on their medical conditions, prescribed medications, and self-care practices
    • Advocate for patients' rights and needs within the healthcare system
  • Stay Current with Best Practices
    • Participate in ongoing education and training to stay current on nursing best practices and advancements
    • Adhere to established protocols and guidelines for infection control and medication management
Requirements
  • Valid and active Licensed Practical Nurse (LPN) State license
  • BLS certification
