Patient Care Coordinator

At PersonalRX, we are seeking a highly skilled and compassionate Patient Care Coordinator to join our team. As a Patient Care Coordinator, you will play a vital role in ensuring the seamless delivery of patient care services.
Key Responsibilities:- Coordinate patient care services, including scheduling mid-month patient calls and reviewing patient profiles to identify potential medication needs.
- Conduct mid-month patient calls, update patient notes, and follow up with patients to collect outstanding balances.
- Update fill lists, contact patients' doctors to obtain new prescriptions or renewals, and notify patients of receipt.
- Develop strong relationships with patients, becoming an integral part of their care team.
- Enter patient contact information, update patient notes, and obtain necessary information for prescription refills.
- Answer phone calls, direct calls to pharmacists as needed, and process electronic third-party claims for payment.
- Resolve rejected claims, manage patient-supplied medications, and work with finance and senior management to address patient balances.
- Maintain patient addresses, complete monthly medication reviews, and set tasks to ensure timely follow-ups.
- Review and enhance SOPs and policy procedures as needed, identifying areas for new policies or procedures.
- Pharmacy technician license required.
- Computer proficiency and ability to learn company systems.
