Clinical Leadership Opportunity at VNS Health

**Job Summary:**

VNS Health is seeking a skilled Clinical Supervisor to oversee the Hospice Intake process. As a key member of our team, you will ensure the delivery of high-quality care to our patients while maintaining regulatory compliance and standards of practice.

**Responsibilities:**

* Supervise and coordinate all aspects of the Hospice Liaison Intake process
* Ensure effective triaging of new referrals and admissions
* Collaborate with Quality Improvement/Education to develop and implement quality improvement and educational activities
* Work effectively with administration to design, implement, and evaluate quality initiatives and educational plans
* Communicate effectively with VNS Health Care management and hospice agencies under contractual relationship
* Serve as an effective member of the clinical team, demonstrating teamwork, supervision, and collaboration

**Requirements:**

* License and current registration to practice as a Registered Professional Nurse in the state of New York
* Valid driver's license or NYS Non-Driver photo ID card
* Bachelor's Degree in Nursing
* Minimum of three years of progressive clinical experience in home care, Hospice, or related field
