Primary Care Nurse Practitioner/Physician Assistant

The Nurse Practitioner/Physician Assistant Primary Care ALF will provide primary care services to patients in an assisted living facility. This role requires a collaborative approach to provide high-quality care to patients.
Responsibilities- Conduct initial and ongoing assessments of patients' health status to develop and implement care plans based on their needs and goals.
- Prescribe medications and treatments as appropriate, and order and interpret diagnostic tests to ensure accurate diagnoses.
- Provide patient education and counseling on health promotion and disease prevention to empower patients to take control of their health.
- Collaborate with other healthcare professionals to ensure coordinated and comprehensive care, and maintain accurate and up-to-date medical records.
- Participate in quality improvement initiatives to continuously improve patient care and outcomes.
- Current licensure as a Nurse Practitioner or Physician Assistant in the state of Kentucky.
- Master's degree in Nursing or Physician Assistant Studies.
- Certification as a Nurse Practitioner or Physician Assistant.
- Experience in primary care, preferably in an assisted living or long-term care setting.
- Excellent communication and interpersonal skills to effectively collaborate with patients, families, and healthcare teams.
- Ability to work independently and as part of a team to achieve shared goals and objectives.
