MOMS Home Visitor

Job Summary:
The MOMS Home Visitor is a vital member of our team at Family and Children's Agency Inc, responsible for providing home visiting and case management services to families with children ages birth to eight years old. This role requires a strong understanding of child development, parenting, and community resources, as well as excellent communication and interpersonal skills.
Key Responsibilities:
- Provide home visiting and case management services to a caseload of 10 to 12 families living in the Greater Norwalk Area.
- Facilitate groups and workshops as assigned, and assist with transportation and/or childcare to groups/events as needed.
- Outreach to identify and recruit new clients for the MOMS Program.
- Connect clients with appropriate referrals to community resources, including health and social services.
- Accompany clients to medical and agency appointments as necessary.
- Serve as an advocate for clients with schools, health care, and agency providers.
- Provide accurate and relevant information on child development and child health and wellness.
- Identify client goals and work towards the attainment of client goals.
- Assist client in strengthening parenting skills and problem-solving skills.
- Help clients identify and use their informal support network.
- Participate in orientation and training sessions.
- Attend regularly scheduled supervision and staff meetings.
- Attend community and state-wide meetings and trainings as assigned.
- Complete all assigned paperwork in an accurate and timely manner.
- Maintain client records in online database.
Requirements:
- Awareness of cultural and socioeconomic characteristics of clients served.
- Bilingual – Spanish/English.
- Valid drivers license and reliable transportation.
- Knowledge and expertise in the areas of child development, parenting, community resources, outreach, prenatal/post partum health care, and diverse populations.
- Bachelors preferred; Associate's degree in human services, social work, early childhood education or related discipline required.
- Experience facilitating groups preferred.
