Key Responsibilities:

Nutrition Consultations: Address nutrition consultations within one day of request, documenting completion in the electronic medical record (EMR).

Nutrition Evaluations: Conduct evaluations for patients identified at nutritional risk within two days, entering findings into the EMR.



Follow-up Assessments: Reassess the care plan for patients at nutritional risk within one week of the initial evaluation and as needed.

Nutritional Education:
Provide educational resources to inpatients and outpatients to empower them in managing their medical nutrition therapy.



Nutritional Workshops:
Design and facilitate workshops (e.g., cardiac health, diabetes management, weight control) with up-to-date nutritional information for patients, healthcare providers, and nursing students. Ensure education is comprehensible for patients and caregivers, documenting sessions in the EMR within 24 hours.



Outpatient Nutritional Therapy:
Deliver individualized education to outpatients, ensuring clarity and understanding. Provide documentation of therapy to the provider's office within one week.



Community Nutritional Initiatives:
Develop and present relevant nutrition topics based on community needs, coordinating with marketing as necessary and completing required reports.



Committee Participation:
Actively participate in committee meetings, reporting progress to the clinical team and serving as an educational resource for staff.



Clinical and IT Collaboration:
Engage in projects that bridge clinical and information technology, assisting with staff training as assigned.



Phone Inquiries:
Respond promptly to phone inquiries, aiming to return calls within 24 hours.

  • CDR Registration &

State Licensure:
Commit to ongoing professional development to maintain current knowledge, CDR registration, and state licensure.



Departmental Education:
Attend departmental training sessions and meetings, participating in quality assurance and performance improvement programs as assigned.



HealthStream Compliance:
Complete all annual HealthStream modules with 100% compliance.
