Job Summary

The Licensed Practical Nurse (LPN) plays a vital role in ensuring the highest level of physical, mental, and psychosocial well-being for our residents. As a key member of our healthcare team, you will provide direct care, administer medications, and perform quality personal care to meet the unique needs of each resident.

Key Responsibilities
  • Direct Care and Medication Administration: Provide individualized care to residents, administering medications and treatments as prescribed by medical providers.
  • Resident Advocacy: Act as an advocate to maintain resident privacy and dignity during care, ensuring their needs are met and their rights are respected.
  • Monitoring and Communication: Monitor resident vitals, complete focused assessments, and communicate changes to the RN Nurse Manager, LPN Clinical Coordinator, Clinical Lead, medical providers, and family members as instructed.
  • Interdisciplinary Collaboration: Collaborate with the IDT (interdisciplinary team) to promote adequate and appropriate flow of services, ensuring seamless care delivery.
  • Dining Room Support: Assist residents with eating as needed, ensuring their nutritional needs are met.
  • Admission Support: Assist throughout the admission process, ensuring a smooth transition for new residents.
  • Focused Assessments: Periodically perform focused assessments of residents with acute changes in condition, reporting changes to medical providers.
  • Care Conferences: Attend resident Care Conferences as needed, ensuring resident-centered care.
  • Nursing Leadership: Monitor and provide guidance to NARs (Nursing Assistants, Residents) and TMAs (Team Members, Assistants), ensuring they are equipped to provide high-quality care.
  • Work Assignment and Communication: Organize and provide work assignments to unit staff, ensuring clear expectations and effective communication.
  • Documentation and Charting: Maintain and complete all required documentation and charting on the computer or on forms in an accurate and timely manner, adhering to policies and procedures.
  • Medication Administration Record: Accurately chart Nurse's notes reflecting the care provided and resident response, initial Medication Administration Record (MAR) after medication is administered, and document refusals for medication on the back of MAR.
  • Additional Responsibilities: Support various other nursing responsibilities as needed, ensuring the delivery of high-quality care.
Requirements
  • Licensure and Certification: Must be licensed as a Licensed Practical Nurse in the State of Minnesota and maintain licensure.
  • Experience and Education: One or more years of experience in long-term care is highly preferred, with a focus on delivering high-quality care to residents.
  • CPR Certification: Current certification in CPR is required.

  • Computer Skills: Strong computer skills, including Microsoft Word, Excel, Outlook, Internet, HRIS, and other applications, with the ability to manage databases and maintain accurate records.
  • Confidentiality and HIPAA: Understand and maintain the confidential nature of organization and client information, adhering to HIPAA regulations and corporate compliance standards.
  • Customer Service and Communication: Display high levels of customer service, responding promptly and thoroughly to the inquiries and needs of individuals both internal and external to the organization, in accordance with our PROUD standards.
  • Safety and Emergency Procedures: Understand and follow facility and department safety and emergency procedures, ensuring a safe working environment for residents and staff.
Physical Requirements
  • Physical Demands: Perform heavy work, supporting up to 100 pounds, lifting up to 50 pounds with assistance, with occasional lifting/carrying of objects weighing up to 25 pounds.
  • Communication: Effectively communicate in the English language, including following oral and written instructions, to communicate with people inside and outside the organization.
  • Work Environment: Work at a desk for extended periods, perform work at a computer, and move around the department and throughout the facility repeatedly throughout the day.
  • Manual Dexterity: Apply manual dexterity and fine motor skills for computer keyboarding and medical and office equipment use.
  • Vision Acuity: Vision acuity to read fine print, warning labels, doctor prescriptions, etc.
  • Physical Ability: Turn, bend, stoop, kneel, squat, and reach frequently in the performance of nursing responsibilities.
  • Work Schedule: Work a flexible work schedule, including overtime, nights, and weekends as required, including attending events/meetings as needed.
Tools and Equipment
  • Medical Equipment: Use thermometer, stethoscope, blood pressure cuff, wheelchair, patient lifts, and related medical equipment.
  • Standard Office Equipment: Use telephone, computer, and standard office equipment.
Work Environment

The work environment is indoors, with the incumbent required to be flexible about switching positions quickly to respond to the needs of residents and respond to emergencies. May be at risk of exposure to blood, bodily fluids, and other potentially infectious material.
