Medical Care Coordinator

We are seeking a highly skilled Licensed Practical Nurse to join our team at the Northeast Center for Rehabilitation and Brain Injury. This is a rewarding opportunity to provide exceptional patient care and make a meaningful difference in people's lives.

About the Role:
This position involves managing patient care plans, conducting medical interviews, and performing various diagnostic procedures. The successful candidate will also assist physicians and registered nurses, communicate with patients and their families, and administer medications as needed.
Key Responsibilities:
* Plan and manage patient care according to individual needs
* Conduct medical interviews and record patient history and physical condition
* Obtain patient vital signs, including pulse, blood pressure, temperature, and respiration
* Provide routine care for patients, including wound care and preparing specimens for lab courier
* Assist physicians and registered nurses with patient care and administrative tasks
Qualifications:
* LPN license or recent graduate
Compensation and Benefits:
Salary: $60,000 - $80,000 per year, depending on experience
