Key Responsibilities- Assessment and Planning: Conduct comprehensive assessments of patients' nutritional needs, including review of medical history, dietary habits, and laboratory results. Develop and implement individualized nutrition care plans that address patients' specific needs and goals.
- Nutrition Counseling: Provide education and counseling to patients and their families on healthy eating habits, nutrition therapy, and disease management.
- Collaboration and Communication: Work closely with the healthcare team to ensure seamless communication and coordination of care. Participate in interdisciplinary rounds and contribute to the development of patient care plans.
- Documentation and Quality Improvement: Accurately document patient care and outcomes in the medical record. Participate in quality improvement initiatives to enhance patient care and outcomes.
- Education: Bachelor's degree in Nutrition or a related field.
- Experience: Minimum of 5 years of experience as a Registered Dietitian in a clinical setting.
- Certification: Certified Dietitian/Nutritionist (CDN) or eligible for certification.
