Job Summary

The Nurse Specialist - Patient Care Coordinator will be responsible for coordinating patient care services, including developing and implementing individualized care plans, managing patient flow, and ensuring seamless communication among healthcare providers. This role requires strong clinical expertise, excellent communication and interpersonal skills, and the ability to work effectively in a fast-paced environment.

Key Responsibilities
  1. Develop and implement individualized care plans for patients, taking into account their unique needs and preferences.
  2. Coordinate patient care services, including ordering lab tests, scheduling appointments, and facilitating communication among healthcare providers.
  3. Manage patient flow, ensuring timely and efficient delivery of care services.
  4. Provide education and support to patients and their families, empowering them to take an active role in their care.
  5. Collaborate with interdisciplinary teams to ensure comprehensive and coordinated care.
Requirements

To be successful in this role, you must have:

  • A Bachelor of Science Degree in Nursing (BSN) or equivalent experience.
  • 1 year of experience in a related nursing specialty, preferably in a hospital or healthcare setting.
  • BLS certification and RN license in the state of employment or compact.
Benefits

We offer a competitive salary range of $65,000 - $90,000 per year, depending on experience, plus benefits, including health insurance, retirement savings plan, and paid time off.
