Pinnacle Home Care Inc. is seeking a skilled Clinical Communication Specialist to provide high-quality speech therapy services to patients in their homes.

Job Summary

The ideal candidate will evaluate clients' communication abilities, develop treatment plans, and collaborate with an interdisciplinary team to enhance the quality of life for patients.

Responsibilities:
  • Evaluate clients' speech, language, and hearing abilities, recommending strategies to enhance communication and hearing.
  • Conduct initial and ongoing comprehensive assessments, including Outcome and Assessment Information Set (OASIS) evaluations.
  • Develop and revise treatment plans in consultation with physicians as necessary.
  • Collaborate with interdisciplinary team members on care plans and provide speech pathology training to team members as needed.
  • Educate clients and families on speech therapy techniques and related challenges.
  • Document all evaluation data, treatments, and client progress promptly.
  • Supervise Home Health Aides every 14 days when Speech Therapy is the sole discipline involved.
  • Communicate regularly with physicians, ensuring orders are appropriate and addressing any changes in client condition.
Requirements

To be successful in this role, you should have:

  • A graduate degree in Speech Therapy from an accredited program.
  • Licensure as a Speech Therapist in Florida.
  • A minimum of two years of clinical experience as a Speech Therapist.
  • Current BLS Certification.
  • A valid driver's license and insured, reliable vehicle.
  • Strong emotional resilience and physical stamina, including the ability to stand, stoop, and bend frequently.
  • Excellent judgment, communication, and observational skills.
  • Self-motivation with the ability to work independently.
Benefits

We offer a competitive salary, benefits package, and opportunities for growth and development. The estimated annual salary for this position is approximately $85,000 - $110,000, depending on experience.
