Hospital Liaison RN, Home Health Specialist

We are seeking a skilled and compassionate RN Liaison to join our Home Health team at Sutter Health. As a Hospital Liaison RN, you will play a critical role in facilitating seamless transitions from inpatient care to home health services.
Key Responsibilities- Assess patient needs and develop effective care plans in collaboration with the healthcare team.
- Provide inpatient supportive care and coordinate home health services to ensure a smooth transition.
- Communicate effectively with patients, families, and healthcare providers to ensure continuity of care.
- Collaborate with the interdisciplinary team to ensure timely and efficient care delivery.
- Document patient information and referrals in the electronic medical record.
- Graduate from an accredited school of nursing.
- Current RN licensure in California.
- Basic Life Support (BLS) certification.
- Valid driver's license and automobile insurance.
- 5 years of recent relevant experience in home health or hospital settings.
- Critical thinking skills and in-depth knowledge of clinical disease entities common to hospice/home health and palliative care.
- Ability to function independently and as a member of an interdisciplinary team.
- Effective written and verbal communication skills.
- Ability to maintain harmonious and constructive relationships with internal and external customers.
This position requires the ability to work in a mentally and physically demanding environment. The RN Liaison will be required to work variable shifts, including days, evenings, and weekends, as needed.
