Overview

The Nurse Practitioner Cardiac Service Line Specialist plays a vital role in delivering high-quality patient care in a dynamic and collaborative environment. This position requires a skilled and compassionate Advanced Practice Nurse to work closely with the healthcare team to assess, plan, and implement care for individuals with complex health needs.

Responsibilities

  • Provides comprehensive medical assessments, treatments, and clearances for patients, team members, and external customers.
  • Delivers clinical care that reflects the highest standards of professionalism, customer advocacy, and role modeling.
  • Develops and implements individualized plans of care that meet the unique needs of each patient.
  • Facilitates patient education and support programs to promote health and wellness.
  • Evaluates current protocols and standards of care to ensure best practices are translated into clinical practice.
  • Applies evidence-based practices to achieve positive outcomes for patients.
  • Prescribes medications and recommends over-the-counter medications as appropriate.
  • Initiates laboratory tests, immunizations, and diagnostic tests/procedures as needed.
  • Collaborates with physicians and consults with other healthcare team members to ensure comprehensive care.
  • Provides accurate, systematic, and organized documentation of findings, recommendations, and education.
  • Participates in continuing education and maintains documentation of personal development goals and objectives.
  • Provides culturally sensitive and educationally appropriate education to patients and families.
  • Serves as a clinical resource to nursing staff, patients, and other multidisciplinary team members.
  • Participates in policy/procedure reviews to ensure current evidence-based and value-based practices are reflected.
  • Acts as a resource to staff for addressing clinical, process, and system opportunities for improvement.
  • Promotes a collaborative work environment that supports the organizational mission, behavior expectations, and values.
  • Promotes innovation within clinical practice by seeking opportunities to bring new clinical knowledge into practice.
  • Identifies ethical care issues and coordinates interdisciplinary collaborative meetings to resolve those issues.
  • Floats to various clinic locations as needed.
  • Other duties and/or projects as assigned.

Qualifications

  • Graduate of an NLN/AACN accredited program in nursing.
  • Masters Degree.
  • Maintains clinical competencies through participating in continuing educational programs and clinical practice as mandated by the Board of Nursing and national certification body.
  • Experience in relevant patient care setting or as regulations dictate.

Preferred Qualifications

  • Masters Prepared Advanced Practice Nurse.
  • Minimum of 3 years professional experience.
  • Strong organizational and computer skills.
  • Effective verbal and written communication skills.
  • Emotional intelligence, critical thinking, and effective interpersonal skills.
  • Ability to adapt to multiple and changing priorities.
  • Valid Driver's license.

Licenses and Certifications Required

  • NJ State Professional Registered Nurse License.
  • Advanced Practice Nurse Certification (with Masters Degree).
  • AHA Basic Health Care Life Support HCP Certification.

Licenses and Certifications Preferred

  • CPR Certification.
