Job Summary

We are seeking a skilled Physical Therapist to join our team as a Community Care Specialist. In this role, you will make a powerful impact on your local community through inclusive physical therapy treatment.

The ideal candidate will have a Diploma from a CAPTE-approved Physical Therapy Program and a Physical Therapy state license (or in process). A dedication to providing exceptional quality of care to each patient is essential.

Key Responsibilities

  • Develop and deliver personalized plans of care for diverse patient populations with orthopedic and neurologic diagnoses.
  • Objectively measure patient outcomes using cutting-edge software.
  • Efficiently document evaluations, treatments, re-evaluations, and discharge notes.

Compensation and Benefits
