Key Responsibilities- Deliver high-quality patient care and education in a home health setting, tailoring your approach to meet the unique needs of each patient.
- Conduct thorough medical assessments and develop well-structured care plans that address the physical, emotional, and social needs of your patients.
- Collaborate effectively with healthcare teams, including physicians, therapists, and other professionals, to ensure seamless care delivery and optimal patient outcomes.
- Maintain accurate and up-to-date patient records, ensuring that all relevant information is readily available for healthcare providers and caregivers.
- Participate in quality improvement initiatives, contributing your expertise and insights to enhance the overall quality of care provided by TotalMed.
- Current RN licensure in California is required for this position.
- Valid BLS certification is also necessary, as well as experience working in home health settings, although this is not mandatory.
We offer an estimated salary range of $120,000 - $150,000 per year, depending on experience and qualifications. This competitive compensation package includes benefits such as health insurance, retirement plans, and paid time off.
