Job Summary:

South Shore Health is seeking a skilled Registered Nurse to join our team as a Community Health Nurse. This role will provide nursing care to patients in the community setting, working closely with patients, families, and caregivers to develop and implement individualized plans of care.

Key Responsibilities:

Develop and implement individualized plans of care for patients in collaboration with patients, families, and caregivers.
Perform admission physical and psycho-social assessments to determine appropriate plan of care.
Update plans of care on an ongoing basis as goals are met and problems are identified.
Demonstrate knowledge of department policies and procedures for administering and documenting medications.
Reconcile medications with each visit and review medication profiles with changes as appropriate.

Requirements:

Graduate of an accredited School of Nursing.
Bachelor of Science degree in nursing from a college/university accredited by the NLN is preferred.
One to two (1-2) years of medical/surgical nursing experience is preferred.
Write, speak, understand English. Basic Nursing competencies.

Work Schedule:

Monday - Friday 8am - 4:30pm
1 weekend every 4 weeks
2 holidays per year
