About the Role

Rochester Regional Health is seeking a skilled RN Case Manager to join our Home Care team. As a RN Case Manager, you will be responsible for delivering high-quality, patient-centered care to individuals in their homes.

Key Responsibilities
  • Develop and implement comprehensive plans of care for patients, in collaboration with healthcare teams and patients' families.
  • Conduct thorough assessments of patients' needs and develop strategies to address complex health issues.
  • Coordinate and manage patient care, including clinically complex cases, to ensure efficient and effective delivery of services.
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination.
  • Document patient care and coordinating activities in accordance with agency standards.
Requirements
  • AS degree in Nursing required; 1 year of nursing experience preferred.
  • Prior home health, clinical, and direct patient care experience preferred.
  • Ability to work independently and as part of a team.
