Healthcare Support is seeking a skilled and compassionate Registered Nurse (RN) to join our team as a Home Health Hospice Contract Nurse. This is a fantastic opportunity for a dedicated professional to provide high-quality patient care in a dynamic and supportive environment.
Key Responsibilities- Provide comprehensive nursing care to patients in their homes, focusing on their physical, emotional, and spiritual needs.
- Develop and implement individualized care plans in collaboration with patients, families, and healthcare teams.
- Conduct thorough assessments, identify care needs, and coordinate services to ensure seamless care delivery.
- Collaborate with interdisciplinary teams to ensure continuity of care and address complex patient needs.
- Document patient progress, outcomes, and care plans accurately and thoroughly.
- Active RN license in Ohio or Compact State.
- At least one year of recent Home Health Hospice experience.
- Experience with HCHB charting and IDG meetings/notes.
- Admissions experience and BLS Certification from the AHA.
