Comprehensive Patient Care Specialist

About the Role:
- Provide comprehensive patient care, including routine check-ups, diagnostic testing, and treatment plans.
- Participate in quality improvement initiatives, population health projects, and peer review processes.
- Collaborate with other healthcare professionals to achieve shared goals and improve patient outcomes.
Responsibilities:
- Develop and implement effective treatment plans, including medication management and follow-up appointments.
- Maintain accurate and up-to-date patient records, including medical histories and treatment plans.
- Perform diagnostic testing, such as lab work and imaging studies, as needed.
Requirements:
- Master's degree in Nursing as a Family Nurse Practitioner or Bachelor's Degree as a Physician Assistant.
- 1 year of experience in healthcare.
- Valid driver's license.
- Board certified family nurse practitioner (FNP-C) or physician assistant (PA-C), where eligible.
- Active license, American Association of Nurse Practitioners (AANP) or American Nurses Credentialing Center (ANCC) certification and all necessary state credentials.
Salary Estimate: $130,000 - $160,000 per year based on location and experience.
